How to Become a Patient
With Insurace Coverage*
Just follow these three easy steps!

Visit your Primary Care Provider
Your physician will determine if physical or occupational therapy is necessary for your condition.
Get a Referral for Therapy
Your PCP must fill out the required forms and send directly to our office.
Schedule an Appointment for an Evaluation
After the evaluations, a course of treatment will be determined and implemented.
OR
Direct Access
Doctor's Referral NOT Required
This option is applicable if you don’t have a primary care physician and/or intend to pay out of pocket (no insurance).
Call to Schedule an Evaluation
After the evaulation, a course of treatment will be determined and implemented.
Pay for Services Rendered
You are responsible for payment at the time of service.
